Why we built Alvero

Four practices, one bottleneck

I own four dental practices in British Columbia. For years, the thing that scaled worst wasn't clinical capacity, or chairs, or even hiring. It was the front office. Every practice ran on the same hand-driven choreography: someone pulls a report, someone chases a recall, someone notices the AR is aging, someone reconciles the books on a Sunday. That someone was usually me.

You can buy more chairs. You can hire another hygienist. But you can't buy your way out of being the single point of failure for the operation — not with the tools that existed.

We tried every layer on top of the PMS. They all sit on the same hand-driven workflow.

Dentrix, and the dozen tools that bolt onto it, are good at storing what already happened. They are dashboards over a manual process. They tell you the AR is high; they don't clear it. They show you the recall gap; they don't close it. They surface the problem and hand it back to a human — usually the owner — to actually do.

Every "dental AI" product I evaluated was a feature: a smarter reminder, a better form, a slicker patient app. None of them was a workforce. None of them did the work.

What an AI workforce actually means in a dental practice

Alvero is not a feature and not a PMS replacement. It's an operating shell: a coordinated set of AI agents that sit on top of the system you already run and take the operational work off your team's plate.

Concretely, that's three tiers. A 7-agent Bookkeeper AI that reconciles, categorizes, ages AR, verifies EOBs, and drafts your monthly close. A 4-agent Economic Layer that hunts revenue and cuts cost. And a 3-agent Meta-Tier that watches the whole system and makes it smarter every week — safely, with auto-rollback. Underneath sit the operational agents and a two-way Messenger that drafts patient communication but requires human approval on every outbound PHI event.

How Alvero is different: it replaces the bottleneck, not the PMS

The point isn't to rip out Dentrix. Your team keeps charting and booking the way they always have. The point is to delete the human bottleneck around it — the reporting, the recall chasing, the reconciliation, the morning scramble — so the day is already handled before anyone walks in.

Every clinical output still requires a doctor's sign-off. Every outbound PHI message still requires a human's approval. Alvero does the preparation, never the final decision. That line matters, and we built the whole system around it.

What's next — Monday, June 1, 2026

Today, Alvero goes live across all four practices in production. The numbers are real: AR 90+ down meaningfully, chair utilization in the high 80s, and around twenty minutes a day back per provider. We built it inside real waiting rooms because that's the only place software for dentistry should be hardened.
